NEW:

Brianna Nye, DOI: 08/06/2016. Claimant alleges tire damage caused by debris left in road from car accident in intersection of Ocean Beach Highway and Pacific Way.

 

Betty Jo Swarick, DOI: 07/26/2016. Claimant alleges personal injuries sustained as a result of trip-and-fall over uneven pavement in crosswalk at Hudson and 25th Avenue.

 

Patricia Flores, DOI: 07/24/2016. Claimant alleges that a pipe being used to pump water from the Cowlitz River to Lake Sacajawea is not being properly monitored and has flooded her yard, causing property damage.

 

Oregon Mutual Insurance Company, DOI: 07/28/2014. Subrogee of Chace & Spurgeon alleges City caused or allowed the water pressure in its water supply to Chace & Spurgeon PS's property to exceed the regulatory maximum pounds per inch (PSI) standard and said excessive PSI caused the water pipe and attached equipment within property to rupture which leaked water into property, causing damage.

 

DENIED:

Sadie Petersen, DOI: 07/28/2016. Claimant alleges that a city owned tree was not properly maintained, resulting in a tree branch falling and causing damage to her vehicle on Olympia Way.

 

Kevin Henson, DOI: 11/07/2012. Claimant alleges personal injuries sustained as a result of a trip-and-fall over a raised curb outside Vacuums & More.
